Total Correction of Tetralogy of Fallot in an Infant

Total correction is the most common procedure to correct congenital heart diseases.

Steps are:

  • Right ventricular outflow tract obstruction resection
  • VSD closure
  • RVOT reconstruction
  • Postoperative assessment of transesophageal echocardiograph

Tetralogy of Fallot is one of the most common cyanotic congenital heart conditions which requires surgical treatment.

What happens if the child has tetralogy of Fallot?

  • A hole is present between the heart chambers and the valves are narrow.
  • As a result, the blood that is pumped to the rest of the body will contain less oxygen than usual.
  • Also, the narrowed valve decreases the blood that flows to the lungs.
  • Bluish skin (cyanosis)
  • Deep breathing
  • Increased irritability
  • Prolonged crying

Procedure

  • Indicated for children of age 3-6 months.
  • Thickened muscle is removed.
  • VSD is closed by using a patch.
  • Pulmonary valvotomy is done to relieve RVOT.
  • A transannular outflow tract patch is placed for pulmonary valve hypoplasia.
  • A tube graft is placed between the right ventricle and the pulmonary artery.

Complications

  • Heart failure
  • Arrhythmias
  • Artery stenosis
  • Sudden cardiac arrest
  • Regurgitation
